        Terdapat dua aliran besar interpretasi terhadap lirik lagu yang satu ini. Sebagian interpreter meyakini bahwa tema besar dalam lirik needle adalah tentang kecanduan obat-obatan terlarang. Aliran Interpretasi ini mengasosiasikan kata tapeworm (cacing pita) dengan obat terlarang atau drugs. Menurut mereka, lirik needle memberikan kita gambaran bagaimana drugs mempengaruhi penggunannya. Menghisap kehidupan para penggunannya dari dalam. Pada bagian I'm just sitting in my room (aku hanya duduk di dalam kamar ku), with a needle in my hand (dengan jarum di tangan ku), waiting for the tomb (mennunggu kuburan/makam/kematian), of some old dying man (dari seorang tua yang sekarat). Bagian ini menngambarkan bagaiman seseorang sekarat karena overdosis drugs (needle- dikaitkan dengan heroin, karena penggunaan jarum suntik), dan bagaimana drugs membuat para pecandunya lebih cepat tua.
        Aliran kedua meyakini bahwa tema besar dalam lagu ini adalah tentang pemerintah. Kata tapeworm diasosiasikan dengan pemerintah, bagaimana pemerintah menjadi parasit dalam kehidupan kita. My tapeworm tells me what to do (cacing pita ku memberi tau apa yang harus aku lakukan), my tapeworm tells me where to go (cacing pita ku memberi tau kemana aku harus pergi). Bagian tersebut menggambarkan bagaimana kehidupan kita diatur sedemikian rupa oleh pemerintah. Tapi seperti yang sudah-sudah, System Of A Down selalu menyisipkan ambiguitas dalam sebagian besar karyanya, itu yang membuat pencarian terhadap makna dan pesan yang terkandung dalam lirik-lirik lagu mereka selalu menarik buat saya :D
